FI = Federal Taxpayer ID Number 

SL = State License ID Number 

SP = Specialty License Number

UP = Universal Physician Identification Number

XX = National Provider Identifier

0B = State License ID Number

1G = UPIN Provider Number

G2 = Commercial Provider Number

NI = National Provider Identifier

If the UPIN coding is used, the following may be used for physicians without assigned UPINs

INT000 = for each intern

RES000 = for each resident

PHS000 = for Public Health Service physicians

VAD000 = for Department of Veterans Affairs physicians

RET000 = for retired physicians

SLF000 = for providers to report that the patient is self-referred

OTH000 = for all other unspecified entities without UPINs
